You are included in the Settlement Class if you either (1) accessed or otherwise used URMC’s MyChart patient portal between January 11, 2021, and January 11, 2023, or (2) filled out forms on URMC’s public website between January 1, 2018, and June 12, 2023.
Excluded from the Settlement Class are: (a) all persons who are employees, directors, officers, and agents of URMC, the Judge assigned to the Action, and that Judge’s immediate family and Court staff.
YOU MUST COMPLETE A CLAIM FORM IF YOU ARE A CLASS MEMBER AND WISH TO RECEIVE A CASH PAYMENT.
URMC has agreed to establish a Settlement Fund of $2,850,000.000. Once attorneys’ fees, costs and expenses of litigation, Service Awards for the Class Representatives, and the costs of administration are paid, the entire remaining amount will be distributed evenly to all Class Members who file valid claims.
The specific amount of the cash payment will depend primarily on the number of Class Members who claim this benefit.
To start your online claim form, you must login with your Unique ID and PIN. You can find your Unique ID and PIN on the Notice you received. If you are unable to locate your ID & PIN, please contact us by emailing info@URMCWebsiteSettlement.com. Please request your Claim Login ID & PIN by providing the Class Member’s full name and mailing address.

The deadline to submit a Claim Form online is July 21, 2025. If you are mailing your Claim Form, it must be mailed with a postmark date no later than July 21, 2025.
